From 1st December 2014 all General Practices are required to ask patients what is known as the Friends and Family Test. We would be grateful if you could help us to review the quality of the care we provide by answering the questions on the online form. We will publicise our results on a monthly basis.

If we have your mobile telephone number you will receive a text message after practice appointments asking for you to rate your contact. If you can reply it is helpful.
Paper forms are also available at our reception desk. The forms enable patients to rate their visit and add comments about the visit.

Fit Note requests can only be accepted via eConsult for problems already discussed with a clinician. If it is a new period of sickness and you have not seen or discussed this with a clinician, please contact the surgery on 266 5246 to make an appointment.
