Using the NHS app with the new system.

To re-register for online services on a mobile phone/tablet:

  1. Uninstall/remove the NHS app from your device
  2. Restart your device
  3. Re-install the NHS app
  4. Log back into your NHS account

To re-register for online services using a browser:

  1. Remove any bookmarked links to the NHS website
  2. Delete the browser’s history and cache
  3. Close, then reopen browser
  4. Search for the NHS website
  5. Create a new bookmark
  6. Log back into the NHS website

For any issues please visit the NHS App website.

If you continue to have problems getting back into the online services, the surgery may be able to give some assistance.

Healthy Living for People with Type 2 Diabetes

If you’ve been diagnosed with type 2 diabetes, managing your condition is important. Healthy Living is a free online NHS service for people living with type 2 diabetes. It provides key information to help you keep well and is clinically-proven to help you control your type 2 diabetes.

If you are over 18 years old and have been diagnosed with type 2 diabetes at any point, you can sign up for the Healthy Living service. If you care for or are a family member of someone living with type 2 diabetes, you can sign up too.

Registrars Currently At Lane End

Dr. Nyan Zaw is working at the practice as a GP Registrar. He is a qualified doctor now training to become GPs.

GP Net Earnings

NHS England require that the net earnings of doctors engaged in the practice is publicised, and the required disclosure is shown below. However it should be noted that the prescribed method for calculating earnings is potentially misleading because it takes no account of how much time doctors spend working in the practice, and should not be used to form any judgement about GP earnings, nor to make any comparison with any other practice.

All GP practices are required to declare the mean earnings (e.g. average pay) for GPs working to deliver NHS services to patients at each practice.

The average pay for GPs working in this practice in the last full financial year was £108,160 before Tax and National insurance. This is for 1 full time GP, 6 part time GPs and zero locum GPs who worked in the practice for more than six months.

 

Health Checks for Patients Aged 75 & Over

Patients who are 75 and over and have not had an appointment at the practice in the last 12 months can book a Health Check appointment with our Health Care Assistant.

Improving Access to Screening Services for People with a Learning Disability

Some people (such as those with Learning Disabilities) need additional support to access the national screening services such as those for bowel and breast cancer.

Screening is a way of finding and treating changes in your body before they turn into something serious like cancer. 

We want to make sure that you have the information you need to choose if you want to join in with the screening programs.

Your GP will send information about you to the screening programme, so they can offer to help you understand about screening. 

If you want to know more about this, please ask your GP or Practice Manager

Repeat Dispensing –

If you use the same medication regularly, you may be able to benefit from repeat dispensing. This would reduce how often you need to order your prescription. If you would like to see if you can use this service please contact the practice and ask to speak to Sam or Kelly,

NHS Business Services are providing the practice with NHS numbers for patients who may be suitable for electronic repeat dispensing.  This information is obtained from looking at NHS prescriptions. If we think you could benefit from this service, we will contact you to see if you would like to use it. If you would prefer to stay with your usual system for ordering prescriptions that will be fine.

Primary Care Navigators

Primary Care Navigators (PCNs) are highly trained and experienced members of our Practice staff. They can guide patients to local community groups, services and organisations for support, particularly when you don’t know where else to turn and who to ask. PCNs know many of the people involved with local groups and can help find activities that suit your interests. They can help you to speak to the right people to solve problems with benefits, social services and help around your home, and can also help with housing and repairs.

One of our doctors or nurses may recommend you speak to a Primary Care Navigator or you can simply ask at reception to speak to Pauline or Sharon.
