Patient Access can now be used to access Medical Records.
If you already use this service to book appointments and order prescriptions you will need to apply to have access to your medical record. Please ask at reception for an application form.
If you do not currently use Patient Access and are interested in using it please see the Patient Access section on this website, or ask at reception for an application form.
Please note when a child with on-line access turns 13 years old, their account will be closed. It can be restarted, but the child will need to register for this service and complete forms. This is to ensure that the practice follows new guidelines regarding patient confidentiality.
